Which one is the correct tense of the verb?

"I wouldn't have stayed at the hotel if you hadn't recommend it to me." or "I wouldn't have stayed at the hotel if you didn't recommend it to me."

"She would have been hurt in the car accident if she didn't wore her seat belt." or "She would have been hurt in the car accident if she hadn't worn her seat belt."
