accordingly. Number the items 1 to 10.

a. Slide down the [Builder) tab to choose the question format

b. Click on [+Create Survey) to begin you project

c. Add page title by clicking on [Edit]. Type in the [Page Title) box.

d. Sign up to a free online survey tool.

e. Add another [Rating] type question but in this type you need to include both horizontal and vertical rating
scale.



f. Give your survey a title ang choose the appropriate category.

g. Add a [Rating) type question to the form.

h. After designing the form, you are ready to share it to the target respondents,​
